including main indications and safety concerns

Illinois Council of Health System Pharmacists is accredited by the Accreditation
Council for Pharmacy Education as a provider of continuing pharmacy education.
This activity provides 0.5 contact hour (0.05 CEU) of continuing pharmacy
education credit.

Faculty Disclosure:
It is the policy of ICHP to ensure balance, independence, objectivity, and
scientific rigor in all of its education programs. In accordance with this policy,
faculty must disclose to the participants any significant relationships with

Activity Evaluation & CPE Submission Requirements:
In order to receive continuing pharmacy education credit, attendees must complete an activity evaluation and CPE submission online at www.CESally.com. Instructions will be provided during the CPE activity. After successful CPE submission, attendees may print their official CPE Monitor Transcript through their CPE Monitor account at www.NABP.net. For PTCB recertification, CPhTs may use up to 5 hours of P-Specific CE credit hours and CPhT-Adv
up to 10 hours of P-specific credit hours.
