The Saginaw VAMC PGY-1 Pharmacy Residency is an ambulatory care-focused program featuring a wide variety of outpatient learning experiences. Our residency graduates work primarily in PACT or CBOC positions, and many of our recent graduates have stayed on station as our clinical pharmacy services expand and evolve.

All residents participate in a number of clinical activities throughout their program including multidisciplinary rounding, comprehensive patient review, and independent pharmacy dosing services. Ascension IL residents also participate in precepting of pharmacy students and complete a teaching certificate program through Midwestern University.

For more than 100 years, St. Elizabeth Hospital has been a leader in providing quality health care to Fox Valley residents, especially the medically underserved. The hospital meets a broad variety of patient needs through general care, surgical services and numerous medical specialties including cardiology, oncology, pediatrics, emergency medicine, intensive care and obstetrics. With 332 licensed patient beds and more than 420 skilled medical staff, St. Elizabeth Hospital offers complete, compassionate, quality healthcare for the entire family.
The PGY-1 Pharmacy Residency Program at St. Elizabeth Hospital is a 12-month program that is carefully designed following the American Society of Health-System Pharmacists’ Accreditation Standards to ensure that a resident develops into a well-rounded, clinical practitioner through exposure to a large variety of practice areas. Upon completion of this PGY-1 program, the resident will be a competent clinical pharmacist with the necessary professional knowledge and abilities to pursue a general clinical pharmacist position, board certification, and/or a postgraduate year two (PGY-2) pharmacy residency position.
The pharmacy department at St. Elizabeth Hospital provides 24-hour services to all patient care areas. Pharmacists work in a decentralized model on various units including the intensive care unit, cardiology, oncology, neonatal intensive care unit, orthopedics, neurology, psychiatry, and medical/surgical unit. Pharmacists participate in multidisciplinary rounds and readmission reduction efforts, provide clinical dosing and monitoring services, and precept introductory and advanced pharmacy practice students. Our pharmacists provide high quality patient care with assistance from pharmacy technicians and various forms of technology (Pyxis automated medication cabinets, barcode scanning, DoseEdge, and Exactamix.)

All Saints is a 356-bed community hospital located in Racine, Wisconsin. Our comprehensive medical facility integrates the Pharmacy Department in every step of the medication-use process from general medicine to specialty areas including NICU/pediatrics, oncology, medication safety, critical care, , infectious disease, transitions of care, and emergency medicine. Pharmacists at All Saints also participate in all medical emergencies. There are training opportunities for both IPPE and APPE students and the pharmacy department hosts students from the University of Wisconsin, Concordia University, Rosalind Franklin University, Medical College of Wisconsin Pharmacy School, as well as many others. Students and residents are engaged throughout the hospital and will work closely with many other health professionals including physicians, nurses, dieticians, PT/OT, and mid-level practitioners; all from various disciplines and practices.

The program emphasizes progressive pharmacy practice in a community hospital setting. Rotations include: pharmacy practice, internal medicine, critical care, emergency medicine, chronic disease management/transitions of care, antibiotic stewardship, drug policy and administration as well as an opportunities in academia, ambulatory care, informatics and medication safety. Travel includes: ASHP Midyear and the Wisconsin Pharmacy Residency Conference. Presentations at conferences and to nursing, medical and pharmacy staff are expected. All Saints is affiliated with Concordia University School of Pharmacy and residents are offered an opportunity to obtain a teaching certificate during the academia rotation.

St. Francis is a 209 bed community hospital located in Milwaukee, Wisconsin. St. Francis has a de-centralized practice model with pharmacists located in several specialty areas including: ICU, medicine/surgery, and cardiology as well as outpatient oncology and neurology clinics. All pharmacists are ACLS certified and participate in medical emergencies. The pharmacy department trains IPPE and APPE students from several pharmacy schools including the University of Wisconsin, Concordia University – Wisconsin, Medical College of Wisconsin, Midwestern University and Purdue.

The pharmacy residency program is designed to develop independent, insightful practitioners prepared to assume an advanced clinical practice position in a community hospital, pursue a PGY2 in a focused area of interest, or serve as adjunct faculty for a college of pharmacy. Residents completing the program will expand their knowledge and skill in providing optimal pharmaceutical care in various patient-care settings and develop critical thinking and time management skills that will allow them to become self-directed leaders within both their practice setting and the pharmacy profession. Rotations include: Pharmacy Practice, Internal Medicine, Cardiology, Critical Care, Central Pharmacy, Administration/Drug Policy and ambulatory care (neurology and oncology clinics). Elective opportunities include: Academia, Critical Care II, Emergency Medicine, Oncology, Neonatology, and Orthopedic Surgery. Residents completing the Academia elective will have the opportunity to obtain a teaching certificate through Concordia University – Wisconsin School of Pharmacy. St. Joseph Campus has a de-centralized practice model with pharmacists strategically placed in critical care areas (NICU, ED, ICU) as well as the outpatient oncology, infectious diseases, and the general medicine. All pharmacists are ACLS certified and participate in medical emergencies. The site is an affiliated medical education teaching site for the Medical College of Wisconsin. The Pharmacy Department trains IPPE and APPE students from a variety of pharmacy schools. Located in Milwaukee, Wisconsin, the area offers many opportunities to enjoy the outdoors and cultural events.

Residents of the St. Joseph Campus PGY1 Pharmacy Residency Program become skilled clinicians who are self-directed practitioners, prepared to assume advanced practice in an acute care environment. The pharmacy residency program is designed to develop independent, insightful practitioners prepared to assume an advanced clinical practice position in a community hospital, pursue a PGY2 in a focused area of interest, or serve as adjunct faculty for a college of pharmacy.

All residents complete orientation, emergency medicine, internal medicine, pharmacy administration, staffing and a yearlong project. Electives include NICU/Women’s Health, ambulatory care rotations, oncology and advanced rotations in critical care and emergency medicine. The program is individualized to meet the needs of each resident through research project selection, elective rotation offerings, and personalized scheduling. The resident will participate in both site specific and system-level committees and assist with evaluating and improving patterns of drug utilization. Residents may also obtain a teaching certificate through Concordia University Wisconsin School of Pharmacy. A yearlong research project will be presented at the ASHP Midyear Clinical Meeting and the Pharmacy Society of Wisconsin Pharmacy Residency Conference.

Ascension Columbia St. Mary’s Milwaukee is proud to launch a new pharmacy residency program with the first class starting in July 2019.

Ascension Columbia St. Mary’s Milwaukee is a community teaching hospital located in Milwaukee just north of downtown. The Lake Drive Campus is made up of two hospitals, the Main Hospital and Women’s Hospital, and two large clinics, Prospect Medical Commons and Water Tower Medical Commons. Average daily inpatient census is ~225 patients. The facility offers advanced cardiovascular and neurological surgery services, a regional burn center, women’s health services, a neonatal intensive care unit, 48 adult intensive care unit beds, 130 medical/surgical beds. an inpatient behavioral health unit, an inpatient rehabilitation unit, and a 45-bed emergency department. Additionally, an outpatient chemotherapy infusion center is part of the campus. Ascension Columbia St. Mary’s Milwaukee serves as the tertiary care referral center for seven southeastern Wisconsin Ascension hospitals. As a result, the area’s most complex medical and surgical cases come to Ascension Columbia St. Mary’s Milwaukee for care.

Ascension Columbia St. Mary’s aims to develop residents into capable pharmacy clinicians who are prepared to enter an advanced acute care practice environment. Through a variety of rotation experiences, projects, and activities, residents will develop successful time management and communication skills. Graduates will be resourceful, compassionate, and competent members of the healthcare team who demonstrate a commitment to the patient and profession.

Required rotations include: Critical Care, Internal Medicine, Emergency Medicine, Pharmacy Administration, Academia, Research, and Staffing.

The program is a minimum of 24 months. The first 12 months follow the PGY1 Pharmacy program at Columbia St. Mary’s as described above. The last 12 months of the residency year will be completing rotations for the administration portion of the residency. The first six weeks will focus on hospital, pharmacy, and technology orientation. Rotations generally include 4-5 weeks of concentrated training, followed by longitudinal exposure and are designed to integrate both local and system-level training. Longitudinal experiences may be six or twelve months in total duration. The resident may also have opportunities to interact with Ascension National Pharmacy leaders and work on national health-system projects. Time for clinical or administrative electives is offered throughout the year to further develop the resident's skills. Each resident is asked to complete a project that helps contribute to the body of knowledge in pharmacy practice. The scope of the PGY2 project will be applicable to pharmacy practice at the health-system level. The resident will present the final research at the Pharmacy Society of Wisconsin Residency Conference, or equivalent meeting and prepare a final written manuscript. The resident is required to complete orientation the following core rotations: operations, informatics, finance, medication safety, supply chain and formulary management). Elective rotations (340b, ambulatory care management, retail pharmacy management, oncology management, advocacy) and other experiences are tailored to the resident's abilities and interests.

Aspirus Wausau Hospital

Aspirus Wausau Hospital provides primary, secondary and tertiary care services as a regional referral center.  AWH has received recognition as a Primary Stroke Center, Level II Trauma Center, and Level IIIa NICU. The facility has 245 operational beds and an average daily census of 170.

The PGY1 Pharmacy Residency Program at Aspirus Wausau Hospital allows the resident to gain diverse experience in a progressive pharmacy department within the setting of a community-based, tertiary hospital and health-system. A wide variety of acute care opportunities are available to provide care for a population representing the diversity of the region including all levels of care for patients of any age from premature infants to geriatric patients.  Residents will gain experience with glycemic management, total parenteral nutrition, anticoagulation management, and  pharmacokinetics for adult and pediatric patients.  Exposure and involvement in local, regional, and system operational and leadership workgroups and committees will provide exposure and leadership experience at a variety of levels.

Aultman Hospital

The Aultman Hospital Pharmacy Residency Program takes place in a 600 bed teaching hospital in Canton, Ohio that is a level II trauma center, a STEMI receiving center, and a Primary Stroke Center and is affiliated with a federally qualified healthcare clinic. We accept 4 PGY1 residents and 1 PGY2 Emergency Medicine resident each year.

PGY1 residents have the opportunity to select from a wide variety of elective and advanced elective rotations including ambulatory care, cardiology, critical care, emergency medicine, hospice and palliative care, infectious disease, IV room and med safety management, outpatient infusion and chemotherapy, rural health, and teaching. We pride ourselves on making sure our PGY1 resident graduates are qualified and ready to apply for any job or PGY2 program they want to and customize their residency to align with their individual goals.

The PGY1 residency program at Corewell Health William Beaumont University Hospital (formerly known as Beaumont Hospital, Royal Oak) develops residents into confident, competent practitioners. Eight learning experiences are required (Orientation, Ambulatory Care, Critical Care, Drug Information, Integrated Practice Experience, Internal Medicine, Practice Management, Research) with several electives available. The Integrated Practice Experience immerses the resident within the unit-based practice model. An on-call program develops clinical confidence and autonomy in code response, alteplase for stroke, supratherapeutic level evaluation, and antimicrobial stewardship activities. Residents serve as student preceptors throughout the year. A chief resident position, resident chair positions, advisor program, leadership development program and participation in the Wayne State University Teaching Certificate Program are available. At Bronson we strive to prepare pharmacist resident clinicians for inpatient practice in entry level direct patient care positions within hospital settings, including larger tertiary care centers; for adjunct faculty positions; or to pursue post-graduate year two (PGY2) residency training.  Residents complete various required and elective rotations throughout the year to meet predetermined goals and objectives as well as their personal and professional goals. Residents are also exposed to an assortment of teaching opportunities as well as to a variety of volunteer, presentation and project management activities. Residents experience pharmacy practice in a wide variety of practice areas, including acute care, critical care, cardiology, administration, ambulatory care and other specialty areas.

Captain James A. Lovell Federal Health Care Center

Special features: This pharmacy practice residency program enables the resident to participate in structured diverse direct patient care rotations and ongoing experiential activities to achieve the resident's goals.  The resident will be exposed to multiple patient care settings including acute care, ambulatory care, long-term care, Patient Aligned Care Team (PACT), emergency department, medical home port, women's health, pain clinic, oncology, infections disease, home based primary care, psychiatry and many more.  Pharmacy practice skills are enhanced by automation, electronic medical record, formulary management, investigational drug studies and performance improvement projects. Teaching opportunities include co-precepting students, presenting educational programs to allied health professionals and optional didactic lectures at Rosalind Franklin University.  Residents have the opportunity to receive their teaching certificates through Rosalind Franklin University, College of Pharmacy.  A research project is required.

Our programs are designed to encourage residents to develop the knowledge and leadership skills necessary to become leaders in the pharmacy profession. This PGY1 pharmacy residency program has an ASHP candidate status and is accepting two positions to begin July 2024. The position is a 12-month, postgraduate training experience that is designed to expose residents to a wide variety of clinical experiences. Examples of clinical services that will be performed during the residency include, but are not limited to, optimization of medication therapy, patient care rounding, pharmacokinetic and renal dosing, therapeutic drug monitoring, drug information, anticoagulation dosing, and drug interaction monitoring.

In addition to clinical experiences, residents will complete a residency project, earn a teaching certificate through a local university, participate in didactic and experiential teaching, and gain drug utilization evaluation and performance improvement experiences. The goal of this PGY1 program is to prepare residents to provide the highest level of pharmaceutical care while serving as an integral member of the multidisciplinary team. PGY1 Pharmacy Residency
Mount Sinai Hospital’s PGY1 Pharmacy Residency program is a broad, comprehensive pharmacy practice experience that prepares residents for multiple professional challenges and career opportunities. Four PGY1 positions are available; the residency is currently in its 11th year.

Brief Overview of Program
Our Pharmacy Department provides evidence-based, cost-effective and safe drug therapy, targeting optimal patient outcomes; diseases that impact the community (heart failure, diabetes, COPD and asthma) are a focus of the general medicine, transitions of care, and ambulatory care rotations. The PGY1 pharmacy resident can gain experience in areas that include general medicine, critical care, emergency medicine, infectious diseases, antibiotic stewardship, ambulatory care, and pharmacy services management. The faculty includes pharmacy specialists in the areas of MICU, SICU, Emergency Medicine, Infectious Diseases, Oncology, and Transitions of Care. Additionally, experienced general medicine pharmacists and managers are on staff. Expectations of the resident include formal presentations to pharmacists, physicians, medical residents and nurses; conducting MUEs; mentorship of pharmacy students and development and presentation of a project of major significance to Sinai.

PGY2 Health System Pharmacy Administration Leadership Residency
Mount Sinai Hospital’s PGY2 Health System Pharmacy Administration (HSPAL) residency provides hands-on, pharmacy practice experience in multiple areas of pharmacy management; one position is offered. The System Director of Pharmacy Services serves as the RPD. Residents have benefitted from exposure to a wide scope of areas: this PGY2 program has served as a great platform for launching a career in health care administration. Previous residents have obtained positions such as pharmacy manager and pharmacy director. There is a need for the development of strong, future leaders and managers in health system pharmacy. The vision of Mount Sinai Hospital’s PGY2 HSPAL program is to provide a wide range of experiences that will prepare its residents for a future role as a director of pharmacy.

Brief Overview of Program
The resident will have access to a full pharmacy enterprise experience which includes inpatient care, clinical management, ambulatory care services, retail pharmacy, infusion center, 340B/specialty pharmacy, regulatory compliance, business performance, business development, residency programs (will act as chief resident for PGY2 residents), medication safety, strategic planning, regionalizing pharmacy services, APPE/IPP Pharmacy programs, population health and pharmacy revenue cycle optimization. A strong pharmacy operations rotation exposes the residents to personnel management, human resources, information technology automation with other rotations such as 340B drug discount program, budget/finance, medication safety and much more. Training is provided in various aspects of pharmacy management, with emphasis on independent thinking and application of management concepts in the development of progressive pharmacy services. The resident will work with a variety of seasoned and passionate pharmacy managers and will have the opportunity to interact with various leaders across the health system including a rotation with senior leadership, Vice President of Acute Care Hospitals.

PGY-1 Community Based Pharmacy Residency (2 positions)

The PGY1 Community-Based Pharmacy residency program at Northwestern Medicine Specialty Pharmacy builds upon the Doctorate of Pharmacy education to provide a comprehensive post-graduate learning experience to develop community-based pharmacists with diverse patient care, leadership, and education skills who are eligible to pursue advanced training opportunities, including professional certifications and postgraduate year two (PGY2) pharmacy residency training.

Our pharmacy residency program is committed to cultivating exceptional pharmacists through a comprehensive and innovative learning experience. We strive to provide a supportive and collaborative environment, fostering growth in clinical skills, research, and leadership. The purpose is to empower our residents to become confident, patient-centered practitioners whom are dedicated to advancing pharmaceutical care and improving patient outcomes.

The program is 12 months in duration and orientation begins in mid-June. The residency is based at the Northwestern Medicine Specialty Pharmacy (NMSP) of Northwestern Memorial Hospital in Downtown Chicago. NMSP holds both URAC and ACHC accreditation.

PGY-1 Pharmacy Practice Residency (10 positions)

The NMH PGY1 program is committed to prioritizing diversity, equity, and inclusion among pharmacy residents. NMH is a large academic medical center with 897 beds, located in downtown Chicago, Illinois. The pharmacy department at NMH has a long history of training pharmacy residents since 1978. The pharmacy practice model of NMH is a patient-centered, team-based, integrated approach to care. At NMH, every pharmacist has both clinical and operational responsibilities. Clinical pharmacists rotate through day shifts, evening shifts and weekends to provide high level, continuous patient care. We have nearly 200 pharmacists, greater than 50 percent of which are board-certified and greater than 80 percent have completed residency or fellowship training.

PGY-1&2 Health System Pharmacy Administration and Leadership Residency (1 position)

The combined PGY1/PGY2 health-system pharmacy administration and leadership program encompasses a 24-month experience that combines rotational and longitudinal experience in pharmacy practice, management, and administration, as well as longitudinal ambulatory clinic and staffing experiences. This program also includes participation in the ASHP Foundation Pharmacy Leadership Academy, or tuition reimbursement for an external master’s program, throughout the PGY2 year.

The rotation and entrance requirements for the HSPAL PGY1 will be the same as the traditional NMH PGY1 program, but include more administration-focused electives. This will allow the resident to build their clinical skills and competencies through an exceptional PGY1 experience while preparing them for their HSPAL PGY2.  Please refer to the PGY-1 Pharmacy Practice Residency information for full details on the PGY1 program requirements.  During the PGY-2 year, the resident will have the opportunity to learn from multiple departmental and system leaders within Northwestern Medicine, and will get to network with pharmacy leaders across the country through experiences such as elective rotations and the ASHP Pharmacy Leadership Academy.  The PGY-2 resident will also develop interpersonal leadership skills by serving as a chief resident and by leading the pharmacy intern program, giving them the opportunity to grow their management skills with 10 direct reports.


OSF HealthCare Saint Francis Medical Center and the Children's Hospital of Illinois

OSF HealthCare Saint Francis Medical Center (SFMC) and the Childrens Hospital of Illinois is 620 bed teaching hospital located in Peoria, Illinois affiliated with the University of Illinois College of Medicine at Peoria. Three PGY-1 positions annually and one PGY-2 position in Pediatrics annually. SFMC is a Level 1 trauma center, Level 3 NICU, and Comprehensive Stroke Center.

PGY-1 Pharmacy residents will gain comprehensive experience in acute-care practice for both pediatrics and adults. Core rotations include administration, infectious disease, internal medicine, pediatric critical care, and adult medical intensive care. Longitudinal core rotations are pharmacy informatics, drug information, medication safety, precepting, and pharmacy practice (staffing). Elective rotations include: anticoagulation, cariology, neonatal critical care, neurology, nutrition support, oncology, pediatric hematology oncology, emergency medicine, surgical intensive care, and cardiac intensive care. Residents complete a year-long research project. Residents also complete a teaching certificate in conjunction with Southern Illinois University Edwardsville School of Pharmacy. Our goal is to provide an inclusive environment for residents to build upon Doctor of Pharmacy (PharmD) education to provide medication-related care to patients with a wide range of conditions, be eligible for board certification, and eligible for postgraduate year two (PGY2) pharmacy residency training.

PGY-2 Pediatric Pharmacy residents will have the opportunity to practice in the 120 bed Children’s Hospital of Illinois (CHOI), which is part of OSF Saint Francis Medical Center. In addition to a Level 3, nationally recognized NICU, the hospital also has Peoria, IL’s only pediatric specific emergency department. CHOI is also the St. Jude Children’s Research Hospital Midwest Affiliate. Core rotations include: congenital heart unit, pediatric intensive care I and II, neonatal intensive care I and II, pediatric hematology and oncology, nutrition support, pediatric infectious disease, and general pediatrics. Longitudinal core experiences include: pediatric pharmacy practice (staffing), pharmacy administration I and II, and medication safety. Elective rotations available include pediatric emergency room, outpatient hematology and oncology, and residents may repeat any experience they would like as they prepare for their future employment. PGY-2 residents will also complete a year-long research project and will present it at the Pediatric Pharmacy Association National Spring Meeting. Our goal is to provide a supportive and inclusive environment that prepares the resident to pursue pediatric board certification, and to be a competent and confidant general pediatric pharmacy practitioner as they take the next step in their career.

PGY1 Residency:
The UIC College of Pharmacy – Rockford Campus is pleased to offer a one-year PGY1 pharmacy residency. The program is designed to enhance the skills and knowledge obtained during Doctor of Pharmacy education. By focusing on developing clinical pharmacists prepared to offer medication-related care on multidisciplinary teams, the program provides the advantages of preparation for board certification and PGY2 pharmacy residency training. Under the mentorship and guidance of experienced clinical faculty members, the pharmacy resident will transition from a student to an independent practitioner during the 12-month program.

The PGY1 Pharmacy Residency Program resident must complete six clinical rotations in anticoagulation, drug information, family medicine, HIV/hepatitis C, hypertension, and internal medicine.

PGY 2 Residency in Emergency Medicine:
The UIC College of Pharmacy of Rockford, in conjunction with OSF Saint Anthony Medical Center (SAMC) Rockford, offers this one-year PGY2 residency in Emergency Medicine. This ASHP pre-candidate status residency training program will foster and develop the necessary clinical and didactic teaching and research skills and experience such that upon completion of this residency the resident should be prepared to teach other health care professionals, students and trainees. The focus of training in this residency is in the Emergency Department (ED) where residents are expected to develop the skills to be capable to care for patients with a variety of diseases and disorders and varying acuity levels. During this residency training program, there are multiple Emergency Department settings, as well as a variety of elective training sites to meet the individual clinical training needs of the resident. The faculty serves as one of the largest clinical groups in the medical center and provides a wealth of knowledge and experience. The program will provide ample opportunity to enhance leadership and communication skills. Graduates of this program will be skilled clinical practitioners with experience in didactic and clinical teaching as well as clinical research.

 - The PGY1 residency program at SwedishAmerican allows the resident to experience a wide range of clinical areas tailored to the resident's individual interests. VA Illiana Health Care System

VA Illiana Pharmacy Department Pharmacy Service includes the inpatient and outpatient pharmacies in Danville and an outpatient pharmacy at the Peoria Community Based Outpatient Clinic (CBOC). In addition to these pharmacy locations, Clinical Pharmacy Practitioners (CPPs) work on-site at all VA Illiana facilities, except the CBOC.  Within the Pharmacy Service, approximately 60 employees work to provide exceptional care to our Veterans.  CPPs practice at the top of their pharmacy licenses with prescriptive authority under a scope of practice. Patient Aligned Care Team (PACT) CPPs manage Veterans various chronic disease states, including but not limited to, diabetes mellitus, dyslipidemia, hypertension, chronic obstructive pulmonary disease, heart failure, and tobacco cessation. Mental Health CPPs provide pharmaceutical care across the entire spectrum of the Mental Health Service., including outpatient psychiatric clinics, substance use disorder, and acute psychiatric care. Other areas involving CPPs include Pain Management, Home Based Primary care (HBPC), and Long-Term Care. Many of our CPPs have obtained board certifications, and most are residency trained.

Our ASHP-Accredited PGY1 Pharmacy Residency Program is a structured and advanced education and training experience that provides the foundational skills required to become competent and confident clinical practitioners, as well as future leaders in pharmacy practice.
