Register online below. Registration deadline is Friday, April 12, 2019 at 9:00 am. This program is free to ICHP members. However, those who do not attend and do not cancel at least 48 hours prior to the program will be charged $50 to cover expenses. Pre-registration is required. Please bring a copy of your email registration confirmation with you to the meeting.

Seating is limited to the first 50 people. Student seating will limited to the first 10 students.

Learning Objectives for Pharmacists:

1. Discuss key elements for Transitions of Care programs
2. Explain the workflow of a transitions of care pharmacist
3. Describe the process of creating the service
4. Identify specific interventions that can be incorporated into existing pharmacy workflow to improve transitions
of care
