For more than 100 years, St. Elizabeth Hospital has been a leader in providing quality health care to Fox Valley residents, especially the medically underserved. The hospital meets a broad variety of patient needs through general care, surgical services and numerous medical specialties including cardiology, oncology, pediatrics, emergency medicine, intensive care and obstetrics. With 332 licensed patient beds and more than 420 skilled medical staff, St. Elizabeth Hospital offers complete, compassionate, quality healthcare for the entire family.

The PGY-1 Pharmacy Residency Program at St. Elizabeth Hospital is a 12-month program that is carefully designed following the American Society of Health-System Pharmacists’ Accreditation Standards to ensure that a resident develops into a well-rounded, clinical practitioner through exposure to a large variety of practice areas. Upon completion of this PGY-1 program, the resident will be a competent clinical pharmacist with the necessary professional knowledge and abilities to pursue a general clinical pharmacist position, board certification, and/or a postgraduate year two (PGY-2) pharmacy residency position.

The pharmacy department at St. Elizabeth Hospital provides 24-hour services to all patient care areas. Pharmacists work in a decentralized model on various units including the intensive care unit, cardiology, oncology, neonatal intensive care unit, orthopedics, neurology, psychiatry, and medical/surgical unit. Pharmacists participate in multidisciplinary rounds and readmission reduction efforts, provide clinical dosing and monitoring services, and precept introductory and advanced pharmacy practice students. Our pharmacists provide high quality patient care with assistance from pharmacy technicians and various forms of technology (Pyxis automated medication cabinets, barcode scanning, DoseEdge, and Exactamix.)

All Saints is a 356-bed community hospital located in Racine, Wisconsin. Our comprehensive medical facility integrates the Pharmacy Department in every step of the medication-use process from general medicine to specialty areas including NICU/pediatrics, oncology, medication safety, critical care, , infectious disease, transitions of care, and emergency medicine.  Pharmacists at All Saints also participate in all medical emergencies. There are training opportunities for both IPPE and APPE students and the pharmacy department hosts students from the University of Wisconsin, Concordia University, Rosalind Franklin University, Medical College of Wisconsin Pharmacy School, as well as many others. Students and residents are engaged throughout the hospital and will work closely with many other health professionals including physicians, nurses, dieticians, PT/OT, and mid-level practitioners; all from various disciplines and practices.

The program emphasizes progressive pharmacy practice in a community hospital setting. Rotations include: pharmacy practice, internal medicine, critical care, emergency medicine, chronic disease management/transitions of care, antibiotic stewardship, drug policy and administration as well as an opportunities in academia, ambulatory care, informatics and medication safety. Travel includes: ASHP Midyear and the Wisconsin Pharmacy Residency Conference. Presentations at conferences and to nursing, medical and pharmacy staff are expected. All Saints is affiliated with Concordia University School of Pharmacy and residents are offered an opportunity to obtain a teaching certificate during the academia rotation.

St. Francis is a 209 bed community hospital located in Milwaukee, Wisconsin.  St. Francis has a de-centralized practice model with pharmacists located in several specialty areas including: ICU, medicine/surgery, and cardiology as well as outpatient oncology and neurology clinics.  All pharmacists are ACLS certified and participate in medical emergencies.  The pharmacy department trains IPPE and APPE students from several pharmacy schools including the University of Wisconsin, Concordia University – Wisconsin, Medical College of Wisconsin, Midwestern University and Purdue.  

The pharmacy residency program is designed to develop independent, insightful practitioners prepared to assume an advanced clinical practice position in a community hospital, pursue a PGY2 in a focused area of interest, or serve as adjunct faculty for a college of pharmacy.  Residents completing the program will expand their knowledge and skill in providing optimal pharmaceutical care in various patient-care settings and develop critical thinking and time management skills that will allow them to become self-directed leaders within both their practice setting and the pharmacy profession.   Rotations include: Pharmacy Practice, Internal Medicine, Cardiology, Critical Care, Central Pharmacy, Administration/Drug Policy and ambulatory care (neurology and oncology clinics).  Elective opportunities include: Academia, Critical Care II, Emergency Medicine, Oncology, Neonatology, and Orthopedic Surgery.  Residents completing the Academia elective will have the opportunity to obtain a teaching certificate through Concordia University – Wisconsin School of Pharmacy.  A yearlong research project will be presented at the ASHP Midyear Clinical Meeting and the Wisconsin Pharmacy Residency Conference.

St. Joseph Campus has a de-centralized practice model with pharmacists strategically placed in critical care areas (NICU, ED, ICU) as well as the outpatient oncology, infectious diseases, and the general medicine. All pharmacists are ACLS certified and participate in medical emergencies. The site is an affiliated medical education teaching site for the Medical College of Wisconsin. The Pharmacy Department trains IPPE and APPE students from a variety of pharmacy schools. Located in Milwaukee, Wisconsin, the area offers many opportunities to enjoy the outdoors and cultural events.

Residents of the St. Joseph Campus PGY1 Pharmacy Residency Program become skilled clinicians who are self-directed practitioners, prepared to assume advanced practice in an acute care environment. The pharmacy residency program is designed to develop independent, insightful practitioners prepared to assume an advanced clinical practice position in a community hospital, pursue a PGY2 in a focused area of interest, or serve as adjunct faculty for a college of pharmacy.

The St. Joseph PGY1 Pharmacy program has ambulatory care and acute care tracks.  All residents complete orientation, emergency medicine, internal medicine, pharmacy administration, staffing and a yearlong project.  The ambulatory care track resident is required to complete family medicine, chronic disease management (virtual), and anticoagulation clinic rotations. The acute care track required rotations include critical care, infectious diseases/antimicrobial stewardship, and neonatology/women’s health. The program is individualized to meet the needs of each resident through research project selection, elective rotation offerings, and personalized scheduling. The resident will participate in both site specific and system-level committees and assist with evaluating and improving patterns of drug utilization. Elective opportunities are available for experience in cardiology, geriatrics, ambulatory care rotations, orthopedic surgery, critical care II, emergency medicine II, internal medicine II and oncology. Residents may also obtain a teaching certificate through Concordia University Wisconsin School of Pharmacy.  A yearlong research project will be presented at the ASHP Midyear Clinical Meeting and the Pharmacy Society of Wisconsin Pharmacy Residency Conference.

Ascension Columbia St. Mary’s Milwaukee is proud to launch a new pharmacy residency program with the first class starting in July 2019.

Ascension Columbia St. Mary’s Milwaukee is a community teaching hospital located in Milwaukee just north of downtown. The Lake Drive Campus is made up of two hospitals, the Main Hospital and Women’s Hospital, and two large clinics, Prospect Medical Commons and Water Tower Medical Commons. Average daily inpatient census is ~225 patients. The facility offers advanced cardiovascular and neurological surgery services, a regional burn center, women’s health services, a neonatal intensive care unit, 48 adult intensive care unit beds, 130 medical/surgical beds. an inpatient behavioral health unit, an inpatient rehabilitation unit, and a 45-bed emergency department. Additionally, an outpatient chemotherapy infusion center is part of the campus. Ascension Columbia St. Mary’s Milwaukee serves as the tertiary care referral center for seven southeastern Wisconsin Ascension hospitals. As a result, the area’s most complex medical and surgical cases come to Ascension Columbia St. Mary’s Milwaukee for care.

Ascension Columbia St. Mary’s aims to develop residents into capable pharmacy clinicians who are prepared to enter an advanced acute care practice environment. Through a variety of rotation experiences, projects, and activities, residents will develop successful time management and communication skills. Graduates will be resourceful, compassionate, and competent members of the healthcare team who demonstrate a commitment to the patient and profession.

Required rotations include: Critical Care, Internal Medicine, Emergency Medicine, Pharmacy Administration, Academia, Research, and Staffing.

The program is a minimum of 24 months.  The first 12 months follow the PGY1 Pharmacy program at Columbia St. Mary’s as described above.  The last 12 months of the residency year will be completing rotations for the administration portion of the residency.  The first six weeks will focus on hospital, pharmacy, and technology orientation. Rotations generally include 4-5 weeks of concentrated training, followed by longitudinal exposure and are designed to integrate both local and system-level training. Longitudinal experiences may be six or twelve months in total duration.  The resident may also have opportunities to interact with Ascension National Pharmacy leaders and work on national health-system projects.   Time for clinical or administrative electives is offered throughout the year to further develop the resident's skills.  Each resident is asked to complete a project that helps contribute to the body of knowledge in pharmacy practice. The scope of the PGY2 project will be applicable to pharmacy practice at the health-system level. The resident will present the final research at the Pharmacy Society of Wisconsin Residency Conference, or equivalent meeting and prepare a final written manuscript.  The resident is required to complete orientation the following core rotations: operations, informatics, finance, medication safety, supply chain and formulary management). Elective rotations (340b, ambulatory care management, retail pharmacy management, oncology management, advocacy) and other experiences are tailored to the resident's abilities and interests.

The PGY2 Ambulatory Care pharmacy residency program at Ascension Wisconsin is designed to provide PGY1 graduates the opportunity to grow beyond generalist practice and further the development of specialized care in areas specific to ambulatory patients.  It is assumed that the resident has already achieved a basic level of competence commensurate with that of a PGY1 Pharmacy Residency.  The PGY2 residency focuses on developing knowledge, attitudes, training and skills necessary to cultivate pharmacists who are competent and compassionate practitioners who are prepared for a role as an ambulatory care pharmacist.

The PGY2 Ambulatory Care pharmacy resident will be located at the Ascension St. Joseph campus with opportunities to rotate at other sites across Ascension Wisconsin.  The Ascension Wisconsin Ambulatory Care Pharmacy Service provides chronic disease management, anticoagulation, population health and specialty services for Ascension Wisconsin Medical Group clinicians and their patients.  Daily activities will include seeing patients, providing phone based or web-based care to patients, reviewing clinic policies for appropriateness, and implementing quality assurance programs for current and new clinics.  The ambulatory care resident will be expected to proficient at working in the clinics they are working with and, from time to time, will be expected to work as a pharmacist in that stetting to understand firsthand the best way to implement policies/procedures that affect staff and to provide professional growth.

Special features: This pharmacy practice residency program enables the resident to participate in structured diverse direct patient care rotations and ongoing experiential activities to achieve the resident’s goals.  The resident will be exposed to multiple patient care settings including acute care, ambulatory care, long-term care, home based primary care and psychiatry.  Pharmacy practice skills are enhanced by automation, electronic medical record, formulary management, investigational drug studies and performance improvement projects. Teaching opportunities include co-precepting students, presenting educational programs to allied health professionals and optional didactic lectures at Rosalind Franklin University.  Residents have the opportunity to receive their teaching certificates through Rosalind Franklin University, College of Pharmacy.  A research project is required.

Decatur Medical Hospital is a 300 bed community hospital in Decatur, IL that serves as a primary teaching site for the Southern Illinois University School of Medicine (SIU-SOM). Memorial also serves as a pharmacy teaching site for the Southern Illinois University Edwardsville School of Pharmacy (SIUE-SOP), University of Illinois at Chicago – College of Pharmacy, and St. Louis College of Pharmacy.

Decatur Memorial Hospital (DMH) is a Level II Trauma Center and Primary Stroke Center. In addition, DMH is known for general surgery, orthopedic surgery, cardiac services, critical care services, maternity services, diagnostic, and rehabilitative services.

Clinical pharmacy programs/services include:

The PGY1 residency at Decatur Memorial Hospital exposes the resident to a community hospital setting while simultaneously preparing them for a future role in patient care or further training. Edward Hines, Jr. Veterans Affairs Hospital is located 12 miles west of downtown Chicago and is minutes away from entertainment, culture, and diversity. Hines offers primary, extended, and specialty care clinics both on the hospital campus as well as in the suburbs in community-based outpatient clinics in Aurora, Hoffman Estates, Joliet, Kankakee, LaSalle, and Oak Lawn. Hines currently manages approximately 500 acute care and nursing home beds in affiliation with the Loyola University-Chicago, Stritch School of Medicine as well as numerous other professional schools in the area including several colleges of pharmacy.  Hines has a history of being at the forefront of medicine by utilizing technology, improving access to care, and providing high quality healthcare to the veterans we serve.

Hines PGY-1 Pharmacy Residency was established in 1999 and is ASHP accredited. The goal of the PGY-1 program is to build upon Doctor of Pharmacy (PharmD) education to develop clinical pharmacists capable of caring for patients both in an ambulatory or acute care setting, earning board certification, and continuing their training through postgraduate year two (PGY-2) residency programs.  The large majority of our Clinical Pharmacy Specialists have a Scope of Practice to provide independent direct-patient care clinical services (disease state/medication management) within the following areas: Acute Internal Medicine, Administration, Ambulatory Care General Medicine, Anticoagulation, Critical Care, Emergency Medicine, Geriatrics (Long Term Care/Home Based Primary Care), Infectious Diseases, Oncology, Mental Health, Nephrology, Nutrition Support, Pain Management, Renal Transplant, Spinal Cord Injury, and Women's Health. HSHS St. Elizabeth's Hospital

Established in 1875, HSHS St. Elizabeth’s Hospital is a community teaching hospital located in O’Fallon, Illinois, 15 miles from St. Louis, Missouri.  St. Elizabeth’s Hospital serves over 40 different medical specialties and is the primary practice site for the St. Louis University Family Medicine Residency program.  St. Elizabeth’s Hospital is a member of the Hospital Sisters Health System (HSHS), a multi-institutional health care system that sponsors 15 hospitals that is based in Springfield, Illinois.  

The St. Elizabeth's Hospital Pharmacy Residency Program is affiliated with the Southern Illinois University Edwardsville (SIUE) School of Pharmacy and offers two PGY-1 positions and one PGY-2 position in Pharmacy Informatics.  The PGY-1 Pharmacy and PGY-2 Pharmacy Informatics Residency Programs of St. Elizabeth’s Hospital and SIUE School of Pharmacy are accredited by the American Society of Health-System Pharmacists (ASHP).  

The PGY-1 Pharmacy Residency Program allows the residents flexibility in designing their activities to meet their interests.  Core rotations are comprised of Administration, Ambulatory Care, Critical Care, Distribution/Orientation, Internal Medicine and Advanced Internal Medicine, Pharmacy Informatics, and Medication Safety.  More than ten elective opportunities are designed to develop advanced skills in a variety of areas and include, but are not limited to Cardiology, Emergency Medicine, Infectious Disease, Oncology, Pain and Palliative Care, Pediatrics, and Psychiatry.  In addition, the program offers a Teaching Certificate in association with the SIUE School of Pharmacy.  Completion of a major research project suitable for publication is required along with a presentation at a Regional Pharmacy Resident Conference.

The PGY-2 Pharmacy Informatics Residency Program is designed to develop the resident into a leader in pharmacy informatics and health information technology.  The resident will provide support to the pharmacy and informatics departments for the 15 hospitals in the Hospital Sisters Health System.  Rotation opportunities include Clinical Decision Support, Operational Informatics, Evidence-Based Medicine, Health System Informatics, Pharmacy Automation, Supply Chain, Vendor Informatics, Medication Safety, Pharmacy Information Systems, and Administration, Quality, and Regulatory Compliance.  In addition, the program offers an elective Teaching Certificate program in association with the SIUE School of Pharmacy.  The resident also will have the opportunity to deliver presentations at regional and national conferences.

PGY1 Pharmacy Residency
Mount Sinai Hospital’s PGY1 Pharmacy Residency program is a broad, comprehensive pharmacy practice experience that prepares residents for multiple professional challenges and career opportunities. Four PGY1 positions are available; the residency is currently in its 11th year.  

Brief Overview of Program
Our Pharmacy Department provides evidence-based, cost-effective and safe drug therapy, targeting optimal patient outcomes; diseases that impact the community (heart failure, diabetes, COPD and asthma) are a focus of the general medicine, transitions of care, and ambulatory care rotations. The PGY1 pharmacy resident can gain experience in areas that include general medicine, critical care, emergency medicine, infectious diseases, antibiotic stewardship, ambulatory care, and pharmacy services management.  The faculty includes pharmacy specialists in the areas of MICU, SICU, Emergency Medicine, Infectious Diseases, Oncology, and Transitions of Care.   Additionally, experienced general medicine pharmacists and managers are on staff. Expectations of the resident include formal presentations to pharmacists, physicians, medical residents and nurses; conducting MUEs; mentorship of pharmacy students and development and presentation of a project of major significance to Sinai.
 

PGY2 Health System Pharmacy Administration Leadership Residency  
Mount Sinai Hospital’s PGY2 Health System Pharmacy Administration Leadership (HSPAL) residency provides hands-on, pharmacy practice experience in multiple areas of pharmacy management; one position is offered.  The System Director of Pharmacy Services serves as the RPD.  Residents have benefitted from exposure to a wide scope of areas:  this PGY2 program has served as a great platform for launching a career in health care administration.  Previous residents have obtained positions such as pharmacy manager and pharmacy director. There is a need for the development of strong, future leaders and managers in health system pharmacy. The vision of Mount Sinai Hospital’s PGY2 HSPAL program is to provide a wide range of experiences that will prepare its residents for a future role as a director of pharmacy.
 
Brief Overview of Program
The resident will have access to a full pharmacy enterprise experience which includes inpatient care, clinical management, ambulatory care services, retail pharmacy, infusion center, 340B/specialty pharmacy, regulatory compliance, business performance, business development, residency programs (will act as chief resident for PGY2 residents), medication safety, strategic planning, regionalizing pharmacy services, APPE/IPP Pharmacy programs, population health and pharmacy revenue cycle optimization.  A strong pharmacy operations rotation exposes the residents to personnel management, human resources, information technology automation with other rotations such as 340B drug discount program, budget/finance, medication safety and much more.  Training is provided in various aspects of pharmacy management, with emphasis on independent thinking and application of management concepts in the development of progressive pharmacy services.  The resident will work with a variety of seasoned and passionate pharmacy managers and will have the opportunity to interact with various leaders across the health system including a rotation with senior leadership, Vice President of Acute Care Hospitals, Sinai Chicago.

Rush has had an ASHP accredited pharmacy residency program in place for 40 years. Currently there are 8 PGY1 residents and now 2 nontraditional PGY1 residents. Rush is a not-for-profit academic medical center located just west of downtown Chicago in the Illinois Medical District. Core rotation experiences occur in ambulatory care, critical care, antimicrobial stewardship, internal medicine, management, and pediatrics. The program is flexible to allow individuals to pursue elective rotations in specialized areas, such as emergency medicine, transplant, informatics, and oncology. Additional skills are developed through Pharmacy Grand Rounds presentations, completion of a Pharmacy Research Seminar series, and participation in the Pharmacy Education Development and Lecture Series teaching certificate program. Additional requirements include ACLS/PALS certification, medical emergency response, and a medication use evaluation. PGY2 Ambulatory Care Pharmacy Residency

Residency description: Spectrum Health's Postgraduate Year Two (PGY-2) Ambulatory Care Residency is a primary care focused residency that aims to enhance training  gained through the Doctor of Pharmacy (Pharm.D) educational experience and processes gained through a PGY1 pharmacy residency program allowing the development of clinical pharmacist skills in a specialized area of practice. Residents who successfully complete PGY2 pharmacy residency programs are prepared for advanced patient care practices, academic, or other specialized positions, along with board certification. The UIC College of Pharmacy at Rockford is pleased to offer a one-year PGY1 pharmacy residency. The program is designed to solidify the skills and knowledge obtained during Doctor of Pharmacy (Pharm.D.) education. By focusing on developing clinical pharmacists prepared to offer medication-related care on multidisciplinary teams, the program provides the advantages of preparation for board certification and postgraduate year two (PGY2) pharmacy residency training. Under the mentorship and guidance of experienced clinical faculty members, the pharmacy resident will transition from a student to an independent practitioner during the 12-month program.
The UIC Rockford PGY1 resident is required to complete rotations in emergency medicine, internal medicine, HIV/hepatitis C, family medicine, hypertension, and drug information. The resident will also have the opportunity to select from five elective rotations based on their professional interests and career goals. In addition, the resident will be exposed to numerous teaching opportunities and research experiences. Upon graduation from the program, the resident will be prepared to provide exceptional pharmaceutical care in a variety of health care settings.

The UIC College of Pharmacy of Rockford, in conjunction with OSF Saint Anthony Medical Center (SAMC) Rockford, offers this one-year PGY2 residency in Emergency Medicine. This ASHP candidate status residency training program will foster and develop the necessary clinical and didactic teaching and research skills and experience such that upon completion of this residency the resident should be prepared to teach other health care professionals, students and trainees. The focus of training in this residency is in the Emergency Department (ED) where residents are expected to develop the skills to be capable to care for patients with a variety of diseases and disorders and varying acuity levels. During this residency training program, there are multiple Emergency Department settings, as well as a variety of elective training sites to meet the individual clinical training needs of the resident. The faculty serves within one of the largest clinical groups in the medical center and provides a wealth of knowledge and experience. The program will provide ample opportunity to enhance leadership and communication skills. Graduates of this program will be skilled clinical practitioners with experience in didactic and clinical teaching as well as clinical research.

The Medical Center:
OSF Saint Anthony Medical Center (SAMC) is a Magnet® recognized 254-bed Level I Trauma, burn center and DNV Certified comprehensive stroke center. SAMC is one of 15 hospitals within the OSF HealthCare System. It has been nationally recognized for its management of strokes and coronary artery bypass graft surgeries. The Department of Pharmacy maintains an Affiliation Agreement with the  University of Illinois Chicago College of Pharmacy.

The Emergency Department:
The Emergency Department at the OSF SAMC is a 24-bed department with an average of 40,000 patient visits annually. The ED treats both pediatric and adult patients. UChicago Medicine PGY1 Pharmacy Residency

The primary goal of this program is to develop residents into independent practitioners and confident pharmacy leaders who possess in-depth therapeutic knowledge and integrated professional attributes. Designed to provide broad exposure to a variety of patient care scenarios in the acute care and advanced sub-specialty settings, our program emphasizes development of skills in patient centered care, clinical and didactic teaching, research methodology, and clinical leadership.
