Description The UIC College of Pharmacy, Departments of Pharmacy Practice and Ambulatory Care Pharmacy Services seek a Clinical Faculty (Open Rank) /Clinical Pharmacist to join our team.

Clinical Faculty responsibilities include the provision of didactic lectures at various Health Science Colleges and serving as a preceptor for student pharmacists, and residents, including the PGY1 Community Residents. Scholarly activities, including presentations and publications, are an expectation as well.
Clinical Pharmacist responsibilities include providing pharmaceutical and direct patient care in two settings. At CommunityHealth, the largest volunteer-based clinic in the nation, the clinical pharmacists provide clinical pharmacy services such as medication therapy management, diabetes care, and blood pressure management. They also provide disease state management under collaborative agreements and create patient-centered comprehensive plans. At Wood Street Pharmacy, the clinical pharmacists provide medication therapy management for patients in the outpatient pharmacy setting. Eligible patients in the pharmacy can be referred for clinical pharmacy services including comprehensive medication reconciliation; disease state monitoring for diabetes, hypertension and asthma; screenings for blood glucose, blood pressure, and body mass index; education/training on various disease states and medical devices; immunization assessment; and tobacco cessation counseling.

Candidates must be a graduate of an ACPE accredited College of Pharmacy PharmD program with an ASHP accredited PGY1 Community Pharmacy residency. Candidates must possess knowledge and skills such that they qualify for a non-tenured faculty position with the Department of Pharmacy Practice at the UIC college of Pharmacy. Level is commensurate with experience. Candidates must be licensed or eligible for Illinois licensure as a professional pharmacist. Board Certification and bilingual (English/Spanish) is highly desired.
