The Ambulatory Care Network meets at noon on the first Thursday of the month. See below for details about joining in!


We are truly excited and energized about the ICHP Ambulatory Care Network! All ICHP pharmacists, pharmacy technicians and student pharmacists with interest in the delivery of non-hospitalized patient care are welcome to join us in a journey to guide ambulatory practice advancement and advocacy efforts in Illinois. Our call to action is to bolster recognition of pharmacy as a critical element of the healthcare team to ensure the health and wellness of those patients we serve, across all healthcare settings.

Network participants share responsibility to assess and identify ambulatory best practices in the state, foster collaborative research with ambulatory care practice sites, and solicit presentations and content for ICHP Spring and Fall meetings.

Description
The Ambulatory Care Network is a group of ICHP member practitioners who are pharmacists, pharmacy technicians or student pharmacists specializing in coordination and/or delivery of non-hospitalized patient care.

Mission and Vision
The mission of the ICHP Ambulatory Care Network is to enhance the care and improve the health outcomes of patients in Illinois by advancing ambulatory pharmacy practice collaboratively in the non-hospitalized setting. The vision of ICHP’s Ambulatory Care Network is to support state-wide ambulatory pharmacy best practices that ensure optimal medication use for patients in Illinois within patient-centered team based care.

Responsibilities
  1. Provide communications and benefits to ICHP’s ambulatory care members.
  2. Participate in ICHP divisions and committees.
  3. Establish practice, social and community service activities for ICHP’s ambulatory care members.
  4. Create statewide networking opportunities for ICHP’s ambulatory care practitioners.
Format/Activities
  1. Meets via conference call once monthly or as needed.
  2. Possible activities:
    1. Assessment of ambulatory care in the state
    2. Resources for ambulatory care pharmacists- ex: toolkits, how to prepare for board certification
    3. Perform gap analysis of ambulatory care best practices in Illinois
    4. Foster collaborative research with ambulatory care practice sites.
    5. Create ambulatory care programming for fall and spring meetings
    6. Oversee the ICHP Ambulatory Care Network community on ICHPchat.
