Call for Presentations Proposal Form

Members of the Illinois Council of Health-System Pharmacists are invited to submit a proposal for the ICHP 2016 Annual Meeting, taking place September 15-17, 2016 at the Drury Lane Conference Center in Oakbrook Terrace, IL. Below is a list of potential presentation topics of particular interest, however proposals covering all topics related to health-system pharmacy will be considered.

The ICHP Education Meeting Planning Committee will review the proposals and notify the selected presentations/individuals by end of April. Presentation submittals not selected for this meeting will be retained and considered for future meetings.
